Enthusiastic and compassionate Social Care Support Officers to work within the MHLDA Integrated Discharge Team within Adult Services at Lancashire County Council.

Be part of the journey back to independence providing much needed assessments to service users leaving hospital across the county. With your passion and drive for effective care, you will be supporting vulnerable adults and helping put a spring back in their step as they prepare to return home. We need Support Officers who will go that extra mile to ensure our service users are safeguarded, assessing their needs and capabilities constructively. Working alongside your team and other health colleagues you will form lasting and beneficial working relationships with your approachability and eagerness to succeed.

What we are looking for…

Empathy & Resilience – You will be hands on in this role visiting patients in hospital and become a valuable advocate with a person-centred approach – we want you to empathize with each individual and put their welfare as first priority ensuring they are able to regain their freedom in a safe and appropriate way – by undertaking Care Act and Mental Capacity Assessments, you will need to utilize your superb judgment to ensure the correct support is provided – you will also be attending multi-disciplinary meetings to plan suitable care for each individual, remaining resilient in order to obtain the right level of support

challenges – This is a fast paced role with an abundance of varied cases all over the county; you will need ample self-motivation and be able to think on your feet – there may be cases where you will be required to perform virtual assessments for out of area patients where it is not possible for a face to face visit – this will be a challenging aspect to your work as you will need to make informed decisions without actually meeting the service user in person – you will need to base your conclusions on excellent intuition and remain meticulous whilst performing assessments to ensure nothing is missed that may prove detrimental to the individual – we want you to be confident and flexible throughout your daily tasks, being able to prioritize effectively and willing to travel around the county at short notice

collaborative – Although there is an element of working on your own, you will need to be a team player and work well with your colleagues – we want you to be collaborative and use your excellent communication skills to build good working relationships amongst the team and other health Professionals – this will begin with your induction, getting to grips with your new role and meeting your supervisors and colleagues – we provide worthwhile work shadowing and offer regular supervisions with our senior social workers, ensuring your knowledge and skills go from strength to strength – we also have a Social Worker Apprenticeship Scheme and Social Care Academy so you can develop while working and turn your job into a career
